To order your continuation sick note fill out the form below.
If you need the note to cover you for a specific period of time please state this in the 'Additional Information' section. If your doctor does not appear under the 'Your Usual Doctor' section, please select one of the GPs and we will make sure that the message gets directed to the correct doctor. Alternatively, you can write the name of the doctor that you wish to deal with your request under the 'Additional Information' section.
Sick notes are done at the discretion of the GP and you may be asked to make an appointment. Once the form has been submitted please telephone the surgery after two working days to see if your note is ready for collection. Fields marked with an * must be completed.
Please check the terms and conditions before using the online services. Please be aware that the online services will be out of use between the 5th - 10th of January.
