We are very lucky to have a multi-skilled and experienced clinical team that are able to manage chronic illness with our patients.
Chronic Illness’ include conditions such as: Asthma, Diabetes, COPD (chronic obstructive pulmonary disease), CKD (chronic kidney disease), hypothyroidism, and lots more.
Patients with a chronic illness may need regular checks to monitor their health and wellbeing. This may involve certain tests, such as a blood test, a Blood pressure check and sometimes an ECG (echocardiogram) to check their heart.

If you think that you may have an undiagnosed Chronic Illness, please contact the surgery for an appointment with a doctor. The doctor will then arrange the appropriate tests if required.
If you already have a diagnosis your doctor or nurse will be able to advise you of which tests you need and how frequently you will need to be tested.

We offer regular appointments for blood pressure management where we give lifestyle advice, arrange review or refer back to your GP as necessary.
Our Practice Nurses offer annual check ups to known asthmatics and can advise on, review and change treatment as necessary. If you are asthmatic and are not being seen in the asthma clinic please make an appointment. Find out more about your Asthma at the National Asthma Campaign website
Coronary Heart Disease
An annual appointment is offered where lifestyle advice is given, drug therapy monitored and blood tests reviewed.
