Want to save yourself a telephone call or visit to the surgery? Then why not register to use our online services.

Once you are registered, you will be able to check, book and cancel your GP appointments and order any repeat medications you may need. You can also apply to have access to certain elements of your medical records.

You can book GP appointments through our online service (with the exception of medicals) and also appointments for blood tests. It is important though that these appointments are only booked for blood tests that a Hanham Health GP has confirmed you require. Bookings for all other nurse appointments need to be made with our Reception Team at present.
On application you will also be given the option to register for access to certain elements of your medical records.

How to Register to use the Service

  1. You have to Register with the surgery in person. No-one else can do this on your behalf including family members. Registered carers are an exception to this rule.
  2. Please note that we do not currently register any patient under 16 years of age for online services.

  3. You will need to bring along two forms of ID, one showing proof of address (e.g. Utility bill or Bank statement/letter etc) and one photographic (e.g. passport, driving license, official employment ID). This is to comply with the Data Protection Act.
  4. Your ID will be photocopied and verified by our Reception team and then passed to our administration team. Your registration document that provides you with a unique login will be emailed to you shortly after. You will need these details to create your account and we recommend that you do not share these login details with anyone else.
    There are currently two systems that support our online services – ‘Patient Access’ and ‘Evergreen Life (formerly i-patient). Both systems will allow you to view the same information so please choose the one you find easiest to navigate.

    If you lose or forget your password or your access-ID it is possible for us to re-issue your login details. You will be required to collect these in person and will need your address/ID documents again.
